The Coordinated Assessment System (CAS) is the comprehensive assessment that OPWDD uses for people 18 years or older to identify their strengths, needs, and interests. The CAS provides information used in person-centered planning for the person's care.

OPWDD will look into concerns raised about the CAS Summary. If the person or anyone on their care planning team has questions or concerns about the CAS summary, the Care Manager, or Qualified Intellectual Disabilities Professional can:

After the CAS has been completed by the CAS assessor, it is electronically transferred to the person's record in the OPWDD system, CHOICES.
The Care Manager will review the CAS summaries with the person, his or her actively involved family member or legal guardian, and his or her supports (i.e., residential provider, Consumer Advocacy Board (CAB)).
When the CAS summary has been reviewed and there are questions or concerns identified about the summary, the Care Manager or QIDP must complete the CAS Request for Review Form and send a secure email to: coordinated.assessment@opwdd.ny.gov.
